1. Disclosure requests

To request disclosure, post the designated form to the address below, with the required documentation enclosed. Requests must be sent by post. The Company does not accept requests by phone, fax or e-mail.

2. Requests to correct or terminate use of personal information

To request that your personal information be corrected or its use terminated, post the designated form to the address below, with the required documentation enclosed. Requests must be sent by post.

Copy of certificate of residence or of alien registration card
Copy of one of the following forms of ID: Driver’s license, insurance certificate, certificate of your seal impression, passport

4. Disclosure requests by proxy

If the individual requesting disclosure is the legal proxy of a minor or incapacitated adult, a copy of each document below must be enclosed for proof of legal proxy status. These documents are required in addition to the documents specified in Item 3 above.

i.) Document proving right to act as legal proxy: Copy of family register or of Certificate of Registered Items
ii.) Legal proxy’s proof of identity (one copy of one item below): Copy of one of the following forms of ID: Certificate of residence, driver’s license, insurance certificate, certificate of your seal impression

5. Commission, payment method

Commission: 1,200 yen per application
Payment method: Enclose a fixed-denomination postal order (issued by a post office in Japan) for 1,200 yen along with the request form and documents.

6. Method of responding to disclosure requests

The Company will respond in writing to the applicant address entered in the request form.

7. Usage objective of personal information received in requests to disclose, correct or terminate use of personal information

When the Company receives a request to disclose, correct or terminate use of personal information, it shall only use the personal information received in the request as needed to carry out the request (such as disclosing, correcting or terminating use of the applicable personal information).

8. Non-disclosure of Retained Personal Data

The Company shall not disclose Retained Personal Data when the address entered in the request form doesn’t match the address in the document used as proof of identity; when the right of proxy can’t be verified for a request made by a legal proxy; or when the Company’s Retained Personal Data doesn’t match the information in the request, including cases when the information in the request doesn’t exist among the Company’s Retained Personal Data. The Company shall still receive the designated commission when Retained Personal Data is not disclosed.
